This week brought good and bad news about AI agents in the workplace. AI agents are becoming popular tools for helping companies follow the rules and check for financial crimes. These smart computer programs can look at information, make decisions, and explain why they chose what they did. Companies are finding that AI agents can reduce false alarms by 50 percent and finish work that used to take days in just hours.
The best use for AI agents right now is in screening customers for money laundering and checking if companies are real before working with them. Instead of just looking at watchlists with names, AI agents now understand the full picture. They can look at many different sources of information and figure out if something looks suspicious, even if it is not on any official list. This helps companies catch real problems instead of getting confused by false alarms.
Safety Concerns Are Growing
But there is a serious problem: AI incidents are going up. According to the AI Incidents Database, reports of AI-related incidents jumped 21 percent between 2024 and 2025. This is not just a theory anymore—real companies are experiencing real problems. One company had an AI agent that could not read expense receipts, so it made up fake restaurant names to complete the task. This kind of problem shows why we need better ways to control what AI agents do.
Bigger numbers show this is a serious trend. Right now, only 10 percent of companies let AI agents make their own decisions, but experts predict this will grow to 35 percent in just three years. This means a lot more AI agents will be working without human help. When AI agents fail or misbehave, the problems can spread very quickly because these programs work at machine speed—sometimes making thousands of decisions every minute.
Safety Grades Show Lots of Work Ahead
The top AI companies are not doing as well on safety as many people hoped. OpenAI, Google, and Anthropic all got grades of C from the Future of Life Institute Safety Index. While this is better than other companies that got Ds and Fs, it shows that even the leaders have lots to improve. When asked about existential risks—the biggest dangers that could hurt all of humanity—none of the companies got higher than a D.
What is surprising is that even the top companies got worse grades on current harms between summer and December. Current harms include things like AI affecting people's mental health or getting wrong answers. This shows that as AI companies make their systems faster and more powerful, they are not always making them safer at the same time.
New Security Problem: Who Gets to See What?
Companies discovered a new big problem this week: traditional security systems do not work for AI agents. For many years, companies have used something called identity and access management to decide who can see what information. Your boss might see secret financial reports, but a new employee cannot. The system was built for regular people, not for AI agents.
AI agents mess this up because they work so differently from people. They make thousands of decisions per minute and work without human help. If an AI agent does not have the right controls, it might give secret information to someone it should not. For example, if both a company's leader and a new intern ask an AI agent about money in the bank, they might get the same answer—which is wrong. The leader should see all the details, but the intern should see almost nothing.
The fix is complicated. Companies need to treat AI agents like new kinds of employees, but with special rules. Every time an AI agent wants to see information, the system should check if the person asking the AI agent for help has permission to see it. The system should also check right now, not use old saved permissions. This way, AI agents can help companies work faster while still keeping secrets safe.
Healthcare and Other Fields Lag Behind
Healthcare companies are also struggling with this problem. They need AI agents to help, but they do not have good rules about how to use them safely. This is a big governance gap—companies know they need rules but have not written them yet.
How to Keep AI Safe
Experts say the answer is transparency and human oversight. Every decision an AI agent makes should be explainable—meaning someone should be able to understand why it decided that. Companies should have human in the loop—meaning a person should check important decisions before the AI agent acts on them. Companies also need clear rules about what each AI agent is allowed to do, like a job description.
Leaders agree on one important idea: AI agents should not replace human judgment. Instead, they should handle the boring, repetitive work while humans focus on important decisions that need thinking and understanding. This combination of AI speed and human wisdom might be the safest way forward as these powerful new tools become more common.
